Making a referral - Mountbatten Coordination Centre

Did you know you can refer your patients to Mountbatten's services much sooner in their illness? Thanks to our Mountbatten Coordination Centre, our support is now available to many more people who are thought to be in the last five years of life - whatever their illness or condition. This includes patients with life-limiting conditions such as advanced dementia, heart failure, motor neurone disease, cancer and any other terminal illness.  It also includes patients who are growing increasingly frail and elderly. 

By referring your patient to our Coordination Centre, they will be placed on our Share My Care register (with appropriate consent) and assessed by a Clinical Nurse Specialist (Mountbatten Nurse) to ensure the most appropriate care can be arranged. Our Mountbatten Nurses will work with other health and social care professionals in the community to ensure coordinated care is provided. 

The level of care and support the patient receives will be based on need, but all those registered will have access to a 24-hour phone advice and support line which can provide the necessary reassurance, and potentially prevent an unnecessary hospital admission. Every patient will also be encouraged to complete an Advance Care Plan, which promotes a healthy discussion about their future care and preferences, should they be unable to make those decisions due to ill health or lack of capacity. By having an earlier relationship with patients who have five or fewer years left to live, we can better understand their future needs and wishes, and tailor our services to be more able to meet their expectations. For example, we know that many patients would prefer to die at home, supported by our Mountbatten at Home team.

Referral Criteria

For palliative care

Mountbatten supports people living on the Isle of Wight who may be identified as being in the last five years or so of life, or who have palliative care needs. Our services, which are free of charge, are available for people living with any disease in which a deterioration may be life-threatening. Referrals can only be made by a health or social care professional and should be made by completing a referral form.

For bereavement support

Our bereavement support is available to anyone, of any age, living on the Isle of Wight who has experienced a death, no matter where or how that death has occurred. The person who has died does not need to have been under the care of Mountbatten for relatives or friends to receive our bereavement support. Through our partnership with KissyPuppy, the Sophie Rolf Trust, we also provide bereavement support for children and families. You can self-refer into this service, which means that you can contact us directly so we can find the most appropriate form of support, by calling (01983) 533776. 

For our young adults' transition service

Supporting young people from 14 to 25 years old with terminal or life-threatening illnesses to move from children's to adult's services. 

Referrals from education, social and health care professionals are welcomed. Please ensure you have the appropriate consent in place to make a referral. Self-referrals are also accepted. For information, please call (01983) 217370.

For our Lymphoedema service

The Lymphoedema Service at Mountbatten Hospice serves the whole of the Isle of Wight and accepts non-palliative referrals, as well as palliative referrals. 

This is a nurse and AHP led service for the assessment and management of patients with chronic swelling.  To refer a patient, please download and complete a referral form and send it to: Mountbatten Hospice, Halberry Lane, Newport, Isle of Wight PO30 2ER.
