How can I access Mountbatten Isle of Wight's 24/7 care at home services?

Through our Mountbatten Coordination Centre, we provide personalised support for anyone thought to have five or fewer years to live.

When you’re referred to our Mountbatten Coordination Centre by your GP or health and social care professional, you’ll be placed on our Mountbatten register (with your consent). A Mountbatten nurse will assess your needs and work with other health and social care professionals to ensure you receive the right care.

Every person on our register gains access to our 24/7 telephone advice line, providing:

  • Expert guidance and reassurance at any time
  • The potential to prevent unnecessary hospital admissions
  • Support tailored to your needs

What happens when you call the 24/7 advice line?

When you call our helpline, you’ll speak with one of our trained advisors, who will have access to your medical records through the Mountbatten register. This means we already understand your unique situation and can offer tailored advice. If necessary, we’ll connect you with a Mountbatten nurse for further support.

By choosing our helpline instead of 111, you’ll receive personalised care that can help you avoid unnecessary hospital visits. In an emergency, you should still call 999 or 111.
